The long-running medical drama “Grey’s Anatomy” opened its 23rd season Thursday night on ABC, launching a 13-episode run that shook up hospital leadership and altered the lives of several key surgeons.

The premiere, titled “Collide,” picked up one month after the events of the Season 22 finale, which featured the collapse of the Royer Bridge and the exit of Drs. Owen Hunt and Teddy Altman to Paris. The jump in time allowed Grey Sloan Memorial Hospital to complete work on a luxury VIP suite and confront the fallout from a major collision at Seattle-Tacoma International Airport.

A corporate outsider takes the helm

The episode introduced Seth Norton, a surgeon-turned-businessman portrayed by recurring guest actor James Wolk. Norton, whose company Olympic Regent Health financed the hospital’s new private VIP floor, immediately clashed with Dr. Meredith Grey over patient care and financial priorities. By the end of the hour, Norton was revealed as Grey Sloan’s new chief of surgery, filling the post previously held by Altman.

Showrunner Meg Marinis told Variety that Norton’s appointment was designed to reflect modern economic pressures on medical centers. She explained that she wanted to explore “the upstairs-downstairs of medicine” and observe how existing staff navigate a chief who arrives as an outsider unfamiliar with their interpersonal ties.

Grey, played by Ellen Pompeo, made an appearance in the premiere before heading back to Boston. While Dr. Nick Marsh was absent from the episode, Marinis confirmed to Deadline that Meredith and Nick remain engaged following her Season 22 finale proposal. Marinis said actor Scott Speedman will appear later in the season despite scheduling challenges with his other series, “R.J. Decker.”

Personal crises and career changes

Inside the hospital, emergency operations on victims of the Sea-Tac crash exposed deep fractures and surprises among the medical staff. Dr. Amelia Shepherd and Dr. Toni Chapman argued publicly during surgery over a dying airplane pilot, TVLine reported. The conflict stemmed from Amelia previously sleeping with Cass. However, Amelia later visited Toni at home to profess her love, and the pair reconciled, according to the outlet.

Other surgeons faced severe setbacks. Dr. Jo Wilson struggled with emotional trauma while watching an emergency procedure, prompting her to abruptly tell Dr. Atticus “Link” Lincoln that she had quit her position at the hospital. Moments earlier, Link had taken some of Jo’s leftover pain medication as his struggles with substance abuse resurfaced. Meanwhile, Dr. Simone Griffith made the startling discovery that she is pregnant, creating a personal dilemma after recent encounters with both Wes and Dr. Lucas Adams.

Administrative shifts also hit the training ranks. Dr. Miranda Bailey handed over residency director responsibilities to Dr. Winston Ndugu so she could pursue a master’s degree. The promotion complicates Winston’s secret romantic relationship with intern Dr. Jules Millin, Deadline reported.

What lies ahead for Grey Sloan

With Altman and Hunt in France, Jo walking away, and Dr. Blue Kwan still formally fired following previous disciplinary actions, Grey Sloan faces a notable shortage of working surgeons. Marinis told Deadline that Jo’s absence from regular hospital shifts will continue to play out across upcoming episodes rather than resolving immediately.

Questions also linger about the paternity of Simone’s pregnancy and how Norton’s profit-driven philosophy will alter hospital operations. Outside the flagship program, Marinis and franchise creator Shonda Rhimes are developing a midseason rural Texas spinoff, but exact premiere dates and casting details for that project have not yet been announced.
